PHS 398 Cover Page Supplement Form
The Grants.gov PHS 398 Cover Page Supplement form can transmit the necessary project information required for your application - you should check your program announcement for instructions on the requested data for this form. Information for this form is populated from the 'PHS 398 Cover Page Supplement' form specific Questionnaire in Kuali Research and entering Project Income in the proposal budget (if appropriate). This questionnaire will appear in your proposal when you link a Grants.gov opportunity that utilizes this form and mark it for inclusion. The general 'Grants.gov S2S' Questionnaire will also appear and require answers to support other forms in the opportunity.
This form has two versions that are currently active by Grants.gov (version 3.0 and 4.0) - the only difference between versions 3.0 and 4.0 is the pulled Human Subjects information out of the 4.0 form version into it's own form - Human Subjects/Clinical Trial Information. Below outlines the form instructions and where the associated information is mapped from in Kuali Research.
Instructions
Once a Grants.gov opportunity containing the PHS 398 Cover Page Supplement form is attached and marked as included in your federally-sponsored proposal you can complete the necessary form questionnaire in the Questionnaire panel. The below instructions are how to populate/map the necessary information for each section of the form.
1. Vertebrate Animals Section
Please note: Questions in the Vertebrate Animals section will only map if you've added an Animal Subject Compliance entry to the proposal (hence the "Are Human Subjects Involved?" question is YES in the RR Other Project Information form). If no Animal Subjects Compliance is added in the proposal so the "Are Human Subjects Involved?" question is marked NO in the RR Other Project Information form then this section will not map any responses.

Print/Submit
- After successfully uploading the attachment(s) go to the S2S Opportunity Search tab -> Forms panel
- Check the PHS398_CoverPageSupplement form in the select column and click the 'Create PDF' button
- Also, if this form needs to be included in the submission remember to check the 'Include' checkbox prior to final submission to Grants.gov.
- Upon Print and/or Submission the form will populate with the above form mapping logic.
