Federal Match	Questionnaire	Question ID: 302	Will pull the value entered in the 'What is the Federal Match on this project?' question.
Total from NEH	n/a	n/a	Sum total of the Outright Funds and Federal Match amounts.
Cost Sharing	Budget	n/a	Will pull the Total Cost Share amount from the Budget in the proposal.  If the 'submit cost share' option is unselected in the Budget Settings it will not populate. Please note, currently if you disable the submit cost share at a line item it will still populate in the form.
Total Project Costs	n/a	n/a	Sum total of the Total from NEH and Cost Share amounts.
4. Application Information			See below for each specific field instructions.
Will this proposal be submitted to another NEH division, government agency, or private entity for funding?	Questionnaire	Question ID: 303	Click the Yes to answer if you are an this proposal will be submitted to another NEH division.  Otherwise, click No.
If yes, please explain where and when:	Questionnaire	Question ID: 304	Enter additional details if prior answer was Yes.
Type of Application	Questionnaire	Question ID: 305	Click Yes or No to the 'Is this application for supplemental funding?' question.  If No, then it will select 'New' on form.  If Yes, it will select Supplement.
If supplement, list current grant number(s).	Sponsor & Program Information	n/a	If Yes on prior question fill out current grant numbers in the Sponsor Proposal ID field on the 'Sponsor & Program Information' tab.
Primary project discipline	Questionnaire	Question ID: 306	Select from the available options in the dropdown for the 'Please select a primary project discipline' question.
Secondary project discipline (optional)	Questionnaire	Question ID: 307, 308	Answer Yes of No to the 'Is there a secondary project discipline?' question. If Yes, select from the available options in the dropdown for the 'Please select a secondary discipline' question.
Tertiary project discipline (optional)	Questionnaire	Question ID: 309, 310	Answer Yes or No to the 'Is there a tertiary project discipline?' question. If Yes, select from the available options in the dropdown for the 'Please select a tertiary discipline' question.

Print/Submit

• After successfully uploading the attachment(s) and compliance entrie(s) go to the S2S Opportunity Search tab -> Forms panel
• Check the SupplementaryCoverSheetforNEHGrantPrograms form in the select column and click the 'Create PDF' button
• Also, if this form needs to be included in the submission remember to check the 'Include' checkbox prior to final submission to Grants.gov.
• Upon Print and/or Submission the form will populate like described above with the referenced attachments appended behind the form.
