SFLLL Form
The Grants.gov CD-511 form (aka US Dept. of Commerce Certification Regarding Lobbying) maps information from your proposal data and information maintained centrally for the Organization.
Instructions
Once a Grants.gov opportunity containing the CD-511 form is attached in your federally-sponsored the form information populates per the below mapping.
Name of Applicant: This is the name of the Proposal Organization associated with the proposal. The data comes from Organization record.
Award Number: Award ID referenced on Proposal Details Screen (if included) - the Award ID lookup is only available for certain Proposal Types (not New).
Project Name: Project Title from the Proposal Details screen.
Signature:
Submitted proposals: the approver’s name and title (from the osp$person table) is populated.
Routing/In-Progess proposals: the name and title of organizational contact person.
Print/Submit
- Go to the S2S Opportunity Search tab -> Forms panel
- Check the CD-511 form in the select column and click the 'Create PDF' button
- Also, if this form needs to be included in the submission remember to check the 'Include' checkbox prior to final submission to Grants.gov (if not already marked YES).
- Upon Print and/or Submission the CD-511 form will populate like below.
