New FORMS-I System-to-System Forms for Due Dates on or after January 25, 2025

Forms-I Release

The Forms-I updates to System-to-system forms updates are complete and available for use in funding opportunities with due date on or after January 25, 2025.

NIH Notice - NOT-OD-24-086

NIH Notice NOT-OD-24-086 announced the new “NIH “Forms-I” Grant Application Forms and Instructions Coming for Due Dates on or after January 25, 2025”. Customers can refer to the changes listed in this High-level Summary of Form Changes in FORMS-I Application Packages document.

Effective Date

Applicants must use FORMS-I application packages for due dates on or after January 25, 2025 and must use FORMS-I application packages for due dates on or before January 24, 2025. Applications submitted using the wrong forms for their intended due date may be withdrawn and removed from funding consideration.

Forms Repository (https://www.grants.gov/web/grants/forms/forms-repository.html) 

The forms repository contains copies of the federal forms currently used by awarding agencies to create grant application packages in Grants.gov, forms for post-award reporting, and forms that have been retired from the system. These include both government-wide and agency-specific forms.

Note: We are only updating forms that Kuali currently supports.  A current list of all supported forms can be found in the S2S - Supported Forms article.

• PHS Fellowship Supplemental Form V8.0 - Updated Expiration Date and renamed and reordered the below section/attachment names:
  • Renamed “Fellowship Applicant Section” to “Candidate Section” and renamed “Applicants Background and Goals for Fellowship Training” attachment to “Goals, Preparedness, and Potential”
  • Renamed in the Research Training Plan Section the “Specific Aims”, “Research Strategy”, “Respective Contributions”, and “Selection of Sponsor and Institution” attachments to “Training Activities and Timeline”, “Research Training Project Specific Aims”, and “Research Training Project Strategy”.
  • Renamed “Sponsor(s), Collaborator(s), and Consultant(s) Section” to “Commitment to Candidate, Mentoring, and Training Environment” and renamed “Sponsor and Co-Sponsor Statements” attachment to “Sponsor(s) Commitment”.  Also, the “Description of Candidate’s Contribution to the Program Goals” attachment was moved into this section.
  • The “Institutional Environment and Commitment to Training Section” removed including the “Description of Institutional Environment and Commitment to Training” attachment.
  
• PHS 398 Cover Page Supplement V5.0 - Heading label “6. Change of Investigator/Change of Institution Section” to “6. Change of Investigator/Change of Recipient Organization Section“ and Field label change from “Change of Grantee Institution” to “Change of Recipient Organization”
• PHS 398 Research Training Program Plan V6.0 - Updated Expiration Date and added the “Recruitment Plan to Enhance Diversity” attachment to the Training Program Section (moving this information into a separate attachment means it no longer counts towards the Program Plan page limit.  This will map from the 'PHS_ResTrainingPlan_RecruitPlanToEnhanceDiversity' attachment type in Kuali.
