Grants.gov Form Version Updates for S2S

Overview

The Federal Government updated the SF 424 and R&R form sets used for Grants.gov opportunities and we've added these new form versions to Kuali Research.  Most of these updates were not substantial and were just extending the posted expiration date in the form or related to Country/State code changes.  There were a handful of forms with minor field changes which are outlined below.  These new form versions are available for opportunity deadlines in May and they're currently available if you would like to test in your lower non-PRD environments using test Grants.gov opportunities.

Updated Forms

The majority of the form updates were minor and were simply updating the expiration date in the form or related to changes to the Country/State dropdown options.  More information on how these country/state code updates were incorporated in Kuali Research can be found in the full announcement here.

The only forms that had any changes to fields/attachments or how the forms behaved are listed below and link out to the updated documentation.  Primarily it was removing or reordering fields in the form or adding additional attachment options. 

• PHS 398 Cover Page Supplement v5.0
  • Updated Expiration Date
  • Added Human Fetal Tissue Section including question -
    “Does the proposed project involve human fetal tissue obtained from elective abortions?”
    • If Yes, two new attachments are requested
      1. HFT Compliance Assurance ('HFT_ComplianceAssurance' Narrative ID -5)
      2. HFT Sample IRB Consent Form ('HFT_SampleIRB_ConsentForm' Narrative ID -6)
  • Renumbered form fields, as needed
• PHS Fellowship Supplemental Form v5.0
  • Updated Expiration Date
  • Added new attachment titled “Description of Candidate’s Contribution to Program Goals” to the Institutional Environment and Commitment to Training Section ('PHS_Fellow_InstitutionCandidateContribution' Narrative ID -7)
  • Renumbered form fields, as needed
• PHS 398 Career Development Award Supplemental v5.0
  • Updated Expiration Date
  • Added new attachment titled “Description of Candidate’s Contribution to Program Goals” to the Environment and Institutional Commitment to Candidate Section ('PHS_Career_Candidate_Contribution' Narrative ID -8)
  • Renumbered form fields, as needed
• PHS Human Subjects and Clinical Trials Information v3.0
  • Updated Expiration Date
  • Reworked landing page to allow an answer and supporting explanation for the question “Does any of the proposed research in the application involve human specimens and/or data?” regardless of answer to human subjects involvement question (previously only available if human subjects involvement was no)
  • Human Study Record Attachment changes (not in Kuali Research but in the extracted form you upload in the Human Subjects Compliance entry):
    • Defaulted Clinical Trial Questionnaire question “1.4.a Does the study involve human participants?” to Yes, since study records are only available when the answer to the “Are Human Subjects Involved?” question on the R&R Other Project Information form is Yes
    • Separated “Inclusion of Women, Minorities, and Children” attachment into two attachments – “Inclusion of Individuals Across the Lifespan” and “Inclusion of Women and Minorities”
    • Renamed “Enrollment of First Subject” field to “Enrollment of First Participant”
    • Added “Inclusion Enrollment Report Title” field to the Inclusion Enrollment Report
    • Removed “Brief Summary” attachment
    • Renamed “Narrative Study Description” attachment to “Detailed Description”
    • Added new question and checkbox – “Is this an applicable clinical trial under FDAAA?”
    • Renumbered form fields, as needed
